To get it correctly let´s separate the original Vanilla / Muse releases from the U.S. licensed releases...

Angel Blade エンジェルブレイド has 3 episodes (Vanilla)
Angel Blade Punish エンジェルブレイドパニッシュ has 3 episodes too (Muse),
Angel Blade Ultimate, where Moena (Angel Blade) is fucked by Dark Mother´s creature [in her dream?], is an extra scene of 05:48 mins and avaliable censored only (subbed by Erobeat) as released only with the Angel Blade Box in Japan.

Opposite to this are the U.S. licensed releases:

Angel Blade by Anime18 contains eps. 1+2 of Angel Blade
Angel Blade Returns by Anime18 contains ep. 3 of Angel Blade and a compilation of eps. 1+2 (32:47 mins - to get the amount of a normal DVD with about 60 mins. only)
Angel Blade Punish by Kitta Media contains all 3 eps.

Anything else is the game Angel Blade: Neo Tokyo Guardians by DigiAnime only...
