Megui and handbrake are quite popular although I don't use them personally.
If you want to convert from a DVD or an ISO and it's JAV specific, you might want to take a look at MeguIVit which was made to be simple and produce very high quality for Idols/JAV movies.
If you want to simply convert from one already encoded format to an mkv or mp4, I've made a script which only require you to drag and drop a video on it to accomplish the task(after you've installed the necessary softwares). The link is in my sig as "Re-encoding script v2.07".
