I've not noticed that problem. That's probably because if a watch western porn these days I stick to solo and lesbian stuff.
But that reminds me of the worst part of 80's porn. They would have 3 sex scenes and one non-sex scene all going at the same time, and would switch back and forth from scene to scene. I think you mean fetish movies mostly BDSM in which of course mistreatment is the main part of the plot such as Bondage movies
I can't remember any main movie in porn with such things and I don't believe you're counting GGG movies and the like as main porn

The thing about U.S. porn, especially the "mainstream" crap pumped out by all those studios in sunny, bankrupt Caulifornyaaahhhh is that it's basically infantile in its view and depiction of sexuality. Well I like some of the older western porn, USA stuff usually. I go look for the older porn, 70s' and 80s' stuff, on occasion. Why would I search the Blue Yonder for old and often hard to come by porn when I frequent a board with tons of it hot off the presses?
Storylines, artistic expression and hot sex produced by an industry that was poorly regulated and when the competion was steep. Not all the porn back then was good IMO but there are gems to be found.

Sure, I watch porn for the action. But there is no artistry in todays porn. No drama, no build up, no suspense. Give me a little mental foreplay, tease me a little, ya know? Yes I mainly inted US , but European's production should be considered as well , I'm separating BDSM fetished movies in which violence real or simulated is in fact the real deal a substitution of actual sex, sometimes in there there's no sex at all.
but if you consider many japanese main porn such as the time stop series , they shows what is technically an exploitation , a non consensual sex act.

What countries? It is legal in Germany, Russia,(well that is no surprise), and the USA. I believe it is also legal in Denmark and Sweden. I am really not sure about any other country but besides any muslim religion dominated countries the only one I would not be surprised to see it illegal may be Australia. Australia is very pro-censorship, much to my surprise.
