As others have said earlier, it all comes down to time constraints. If I have the time and have a 2 or 3 hour JAV vid, I can pace myself and make it all the way through the entire vid. I just don't go for the big explosion until I'm ready. Some times I will watch the entire 2 or 3 hour movie, anticipating climax and then I'll go back to the best scene for the payoff. If you're healthy and can stand the anticipation, it's easy and fun to do.
