I have been watching decensored versions of recent titles and all censored JAV IRs after 2015 or so are condomed and all creampies are fakes. I found only a few possible exceptions; all with minor actresses. One needs to go back to the days of AIKA(2014) and Hatsumi Saki(2015) to find censored titles without condoms and real creampies. Even most non-IR censored titles after 2018 or so are condomed.
P.S. The trend was probably set after 2010 or so. Almost all in BDD and BKD series are condomed and creampies, fakes.

If I was paid to come up with some solution for this, I would use some kind of a capsule that opens remotely* (mechanically of course, no explosions etc.). This way, you could control the "bursting" moment precisely, control everything about the "cum" (the amount, the consistency, etc.), just as you would with cuts, and also, with pussy, as compared to mouth creampies, you don't have to hide it much as even in JAV, there is not a lot of studying the pussy after the "cream" comes out, while with mouthfuls the girl can swallow, and the inspection is much easier. There are many fake cum recipe's. If you cram enough it will come out as the penis is pulled out. Another scenario is that the tip of the condom is cut off so that real semen is ejaculated but I see no rationale for this; you would not use a condom if the actress was willing to take the real semen inside her.
Also the exaggerated expressions and actions bespeak the fake nature of the act.

Actually it is not that precise. Often the actor cannot cum fast enough so that the inserted fake cum is smeared onto the penis. The studio wants you to think that the girl is creaming out of excitement but obviously it is just too much of white stuff to be creaming.
