Obviously it's the studio, because it comes out as a commercial DVD that's a for-profit business activity. If it's the girl do it for the fan(s), why hire a cameraman, (usually) a host, editor, and all the people to make a video production out of it? If it's purely a fan-appreciation activity, it'd be just between the girl and the fan(s) and you wouldn't hear or see it except maybe once in a while a really crappy selfie pix or vid leaks out.

I'd imagine it's also a kind of honor a studio gives to the more successful JAV performers. "Here, you have so many fans [you've sold so many DVD's], we will host a fan appreciation event [and we are reasonably confident we can turn a profit selling DVD of you fucking a bunch of losers]."

The real question is are the fans real or hired guns? More likely they are both.
