I like my JAVs to have as little acting as possible, please. That usually means throwing a bunch of guys and girls together and let them have glorious sex. Of course, a little bit of direction is needed, but telling the actors and actresses when to cum is not cool.
That said, that fake photoshot scene in "Sakura's Squirt Tunnel Adventure" really gets me going. I dunno if Sakura Sakurada was acting but it seems that she really didn't know that it was a set up. I also love that one Akibahonpo/Siro-Hame movie where they took a trip to Thailand. In one scene, the crew met up with a fat Thai dude and let the girl have sex with him. Not much of stories but I like those kind of settings.

Not the pregnancy itself but how the conception is made into a JAV. What if the kid later watches it?
There was a "delivery" JAV, you know. Like they filmed the actress giving birth. I think it's too much because it's made as a porn. As for "normal" delivery videos, you can even watch them on youtube.

Well, apparently it's not a taboo subject as I've thought:

Then I guess a "documentary" about a real couple dating, marrying, making a baby, then deliver him/her would be, uh, interesting.
