It took some searching, but I finally found it. JUFD-829 starring Satomi Tsubakiori 椿織さとみ. It was a bit tricky because she only has those pigtails in the last scene.

椿織- Tsubakiori. That’s the first time I have ever heard that name or Kanji. Nice.
