Its easy enough to make a penis or just a penis head (to put on the male performer's real penis) with latex. And the mosaic can hide so much even crappy workmanship will be ok. Even without mosaic the camera angle can easily hide a rubber tube. There's a Syfy TV series about special effects make-up, it's not expensive and not difficult at all to make any body parts with various function, such as squirting slimy goo. Pretty much if the title of the film says gokkun or sperm drink, I would be skeptical. If there's more than a teaspoon of semen per penis (technically we should say testes) then I'd just assume it's fake.

Ok perhaps there's drugs or herbal medicine (I doubt it!) that makes copious amount of ejaculate and there is an urban legend that prostate massage (also called prostate milking) can make a lot of cum. But after all, it seems cheapest and easiest to use special effects.

The reason I tend to assume it's fake is that on so so many JAV, the amount of semen is so pathetic. If the director/producer don't put any attention to it (most JAV focus on the girl).
