No, uncensored titles aren't filmed outside of Japan. The trick is, all porn in Japan is shot uncensored at the start! They skirt around it by being distributed in the US, as well as having the sites such as Caribbeancom hosted on non-Japanese servers. That's why all these uncensored sites have an English side to them, they just so happen to also have a Japanese version, possibly for Japanese living in the US, that's better maintained for "some reason".
