Let me warn first, I have never seen that video.
This is just a translation from Japanese DANDY fan site.
  • 5 girls appears in this DVD.
  • The 5th of 5 is あいの詩 (Uta Aino).
  • Cover photo is also あいの詩 (Uta Aino).
I think koolster is right, but it's not sure.
 
Maybe I can borrow this thread and ask the same thing about the girl in the third scene. The one who blows the teacher at her desk during a test.

Would really appreciate if someone could give me a name on her. Thanks! :)

This list is also　translation from Japanese DANDY fan site.
It is not sure, but must be not unfounded.

1st あすかりの (Rino Asuka)
2nd 夢見ほのか (Honoka Yumemi)
3rd 葉月里彩 (Risa Hazuki)
4th 祇音 (Shion)
5th あいの詩 (Uta Aino)

So, they say the girl in the 3rd scene is 葉月里彩 (Risa Hazuki).
